At Dentistry By Design, we offer an exclusive membership plan that is designed for those patients who are without dental insurance, but value high-quality dental care.
Being an exclusive member allows you to receive your yearly preventative care at a savings, and offers discounts for any additional services that may be necessary to help maintain your oral health.
MEMBERSHIP DETAILS
Annual Membership Dues
INDIVIDUAL PLANS
- Adults - $425
- Children (14 and younger) - $325
PERIO PLAN
- Fee - $300

- No pre-authorizations required.
- No annual maximums.
- No deductible.
- No waiting periods or pre-existing conditions.
- Cosmetic dentistry included.
- Clear Aligner Orthodontics included (Invisalign, ClearCorrect).
- Free consultation for any cosmetic needs.
SERVICES COVERED
Individual Plan
- Unlimited exams - Includes comprehensive, periodic, and any emergency exams.
- All radiographs (x-rays).
- Adult or Child Prohpylaxis (standard cleaning) - 2 per year*.
- Fluoride - 2 per year for children under 16.
- Oral Cancer
*Patients on a 3 or 4 month hygiene protocol will receive the value of 2 standard cleanings credited towards their hygiene visits.
Perio Plan
All other services discounted 10% our usual and customary fee. Services include, but are not limited to:
- Periodontal Therapy/Scaling and Root Planning
- Fillings
- Crowns
- Bridges
- Implants and Implant Restorations
- Cosmetic Dentistry
- Clear Aligner Orthodontics (Invisalign, ClearCorrect)
- Root Canal
- Extractions
- Partial/Full Dentures
